Registering with a GP practice

When you register permanently with the practice you will need to fill out a registration form. If you are on regular repeat medication, you may be asked to arrange a new patient appointment with the doctor to ensure your medication is up to date. 

Registrations forms are available to collect from reception and once returned, can take up to 4 weeks to process. Please ensure you have an adequate supply of medication from your previous practice/pharmacy. 

When you register with us, it’s necessary that you provide proof of identity (e.g. passport or driving licence).

Registering as a Temporary Resident

If you are staying in the area (within our boundary) and need urgent medical attention, you can register as a temporary resident.

For more information on being treated as a temporary resident please contact your nearest GP practice.

Accessing someone else’s information

As a parent, family member or carer you may require to speak on someone else's behalf if needed. Please call reception on 01463 782214 for further advice on how to organise this. 

 

Accountable GP

From 1st April 2015, practices are required to allocate all patients (including children) with a named accountable GP.

The named GP is responsible for the coordination and delivery of all appropriate services, where required based on clinical judgement, to each of their patients.

  • patients do not need to see their named GP when they book an appointment with the practice
  • patients are entitled to choose to see any GP or nurse in the practice
